Required Forms and Documentation
The CSU Helene Fuld School of Nursing uses the Maryland Hospital Association's designated vendor, CastleBranch, to manage student immunizations, compliance documents, background checks, and drug screenings. General information about CastleBranch: www.castlebranch.com
Compliance Document Collection
All admitted full-time and part-time students who wish to qualify for enrollment at Coppin State University must satisfy the University's immunization requirements. Additional program-specific compliance documents are required to complete your admission and also be in accordance with the policies of Coppin State University, the Helene Fuld School of Nursing and the State of Maryland.
The Student Immunization Tracker facilitates the storage and management of students' records. Once you have created your profile on CastleBranch, you will be prompted to submit your records via digital upload, mail, or fax.
The Document Manager allows the secure upload and storage of the following documents required for admission and compliance:
- CPR Certification
- Flu Vaccine (fall semester only)
- US RN License (RNs only)
Background Check and Drug Screen
The HFSON does not require you to undergo a background check and drug screen. However, the hospitals associated with our clinical education programs require criminal background and urine drug screen checks on incoming students to ensure the safety of the patients treated by students in the program. A review typically takes 1-3 business days to complete.
The reviews are conducted by CastleBranch. The HFSON administrators, faculty, and staff do not receive any information of the student's criminal background or drug screen report. If you have any concerns regarding your background check, please contact CastleBranch directly and NOT the School.
