Health Clearance Requirements

Health History Forms

All students are required to have an annual Health History & Physical Form completed by their Primary Care Provider. A copy of the annual Health History & Physical Form may be downloaded, or you can also obtain a copy of the form from the Nursing Center.

• Students are to take the Health History & Physical Form to their respective Primary Care Provider to be completed.
• The completed form must be returned to the
  Coppin State University
  Helene Fuld School of Nursing Community Health
  1st Floor, Room 131
  Health and Human Services Building by August 1.

Students will not be allowed to attend clinical without verification from the Health Clinic that all forms have been completed.

Please keep copies of all documents for your records.

CPR Certification Cards

Your CPR certification is required yearly for all courses having a clinical rotation, you must submit a copy of your card to faculty the first day of class. All cards must be valid through spring semester. Students will not be allowed to attend the clinical practicum without verification of a current CPR certification status.

The American Heart Association Health Care Provider Basic Life Support/CPR Training card will be the only card accepted.

CPR Training is scheduled at Coppin State University in August of each year in the Health and Human Services Building from 8am - 4pm. The cost is $35.00 or $40.00 (Cash & M.O.). Please call (410) 951-3970 to register for the course on or before August 15.
