Throughout the nation two circumstances distinguish youth with significant disabilities from their same-aged nondisabled peers. First, they are likely to be educated in segregated high school settings when their nondisabled peers have moved on to post-secondary educational settings or employment. Second, unlike their nondisabled peers, they are likely to exit school with dim prospects of unsubsidized, integrated employment as an adult. The Baltimore Transition Connection is designed to alter both of these circumstances through the sequential system-wide implementation of an age appropriate, community-based transition model in the Baltimore City Public School System (BCPSS), an urban school system that is committed to improving both school to work transition services and the post-school outcomes of these youth. The Baltimore Transition Connection model will be implemented through the careful redeployment of BCPSS staff, the redesign of curriculum, the cooperation of post-secondary educational institutions, collaboration with adult service providers, and the participation of youth with disabilities and their families.
